Your first visit at FCCI's Orange Park office starts with a phone call to 904.644.0092. Bring your insurance card, photo ID and a full medicine list. You will talk with your provider about your symptoms and history, get a heart exam, and leave with a clear plan, which may include tests.
Booking your visit
Call the Orange Park office Monday to Friday, 8 a.m. to 5 p.m. FCCI welcomes new patients. If your doctor referred you, mention it. If your insurance needs a referral, the team will tell you. You can also use FCCI's appointment request form, but please only share contact details there, never health information.
FCCI offers same day appointments and testing for urgent matters. If your doctor wanted you seen soon, say so when you call.
Before you arrive
- Find your insurance card and photo ID
- Write a list of medicines, doses and how often you take them
- Jot down your symptoms and questions
- Gather recent test results from other doctors if you have them
- Plan to arrive a few minutes early to check in
During the visit
A team member will check your blood pressure, heart rate and weight. Your provider will ask what brought you in, review your health and family history, and listen to your heart and lungs. You may get an EKG in the office, a quick test that records your heart's electrical activity.
This is your time. Ask anything. If something is not clear, ask the provider to explain it another way.
If tests are ordered
Common next steps include an echocardiogram, a stress test, or a vascular ultrasound. FCCI gives written prep instructions for many tests, and they matter. Read them right away. Some tests need fasting or holding certain medicines. Not every test is done at every office, so the team will tell you where to go.
After your visit
Your provider will share results and next steps, which might be reassurance, a medicine change, more testing, or a follow-up visit. FCCI can share results with your primary care doctor. If you need to cancel or change an appointment, FCCI asks for at least 24 hours notice.

When to call 911
A first visit is for questions that can wait a few days. Chest pain or pressure, sudden shortness of breath, fainting, or stroke signs like face drooping, arm weakness or trouble speaking cannot wait. Call 911 instead of driving in or holding out for your appointment. Paramedics can begin care before you reach the hospital.
