Plans with exceptions

A few plans have exceptions. Blue Cross Blue Shield is accepted with the exception of Shands' Gator Care. Humana is accepted with the exception of Humana Gold Plus HMO and Humana Medicaid. FCCI's referring-provider page also notes care for patients with all insurance plans with the exception of Humana Gold.

If you are not sure which version of a plan you have, read the front of your insurance card to the front desk when you call. They can tell you quickly.

Military families and retirees in Clay County can use Tricare for Life, Tricare Prime and Tricare Standard at FCCI. Medicare and Medicaid of Florida are also on the list. If your plan is not listed, call anyway, because the list FCCI publishes is a short list and FCCI says it accepts most major insurances.

Lower costs in an outpatient setting

FCCI was the first in the region to offer catheterization procedures in an outpatient setting, which FCCI says reduces out-of-pocket costs for patients compared with a traditional hospital setting. FCCI also says it offers payment plans if necessary.

We cannot quote prices on this site because costs depend on your plan, deductible and the test or treatment your doctor orders. For an estimate, call 904.644.0092. FCCI also publishes a self-pay fee schedule for patients without insurance.

A good question to ask when you book: will this test or visit be done in the office or at a hospital? Outpatient care in the office can cost less. Questions about any of this? Call 904.644.0092 and talk with a real person at the Orange Park front desk, Monday to Friday, 8 a.m. to 5 p.m.

What to bring

  • Your insurance card and a photo ID
  • A list of your medicines, including vitamins and supplements
  • Any referral your plan requires
  • Names of your other doctors so FCCI can share results with them

Arriving with these items helps your first visit go faster and keeps your billing clean. If anything changes with your insurance between visits, let the front desk know at check-in so your claims go to the right place. Questions about any of this?
